Skater Registration: 7:00pm
Alumni Game 7:30pm to 10:50pm

To register: Please send attached registration form to Barb Dougan, kjbtptd@wowway.com. Registration is limited so please register ASAP.

Waiver and Release Form: All participants must sign the attached Waiver and Release Form prior to participating in the game. Mail signed form to: Barb Dougan, 1427 Canyon Run, Naperville, IL 60565 or bring it to the game.

Full equipment is required for all players.

COST - $20 Donation to the Tomahawks or to the American Cancer Society (Shoot Out for Cancer)
